FIRST VISIT

What to bring:

  • Photo ID

  • Dental Benefit Card

  • List of Current Medical Prescriptions

  • List of medical conditions

If you have dental insurance, make sure to contact our office prior to your appointment to verify your eligibility for the visit. This saves time and allows us to process any claims and verify co-pay amount. Without accurate information, you are responsible for all of the appointment cost.

Plan to arrive 10 minutes early to fill out some required paperwork. These can be completed online prior to your visit. Provide a government issued photo ID and your dental insurance card at the reception desk.

After your first cleaning a comprehensive examination is performed.  The exam entails an oral cancer screening, evaluation of your dental x-rays, an examination of your gums, and analysis of your overall dentition. From the exam the doctor can evaluate any abnormalities including gum disease, active decay, and occlusal wear.

X-Rays

If your previous dentist has taken x-rays and they are diagnostic, you may request that they forward them to our office. If additional films are necessary, they can be taken at our facility.

IMPORTANT: A parent or guardian must accompany all patients under 18 at visits.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
