Payment is due at the time of service. We offer the following options:

• Payment Plans from Care Credit, Complete eligibility application online prior to your appointment.

Insurance

 

Estimated deductible and patient portion is due at the time of service. We will process claims for you. You are responsible for any remaining balance after insurance payment is received and will be notified via mail or phone.
